Birth Certificate = $5.00Death Certificate = $5.00
Marriage Certificate = $10.00
Regular Marriage License (5 day waiting period) = $30.00
Expedited Marriage License (same day) = $30.00 + $50.00 = $80.00
Affidavit of Paternity = $10.00
To request for a new birth certificate or apply for a marriage license, need to make an appointment.
To request for a new death certificate, walk-in.
To request for an existing birth certificate, death certificate or marriage certificate, mail request to the address above or go to Rancare and place inside the drop box provided. Need to provide name and date on document, a self-addressed stamped envelope and proper payment (local check, cashier’s check or money order addressed to “Treasurer of Guam”).

Due to the recent amendment to Public Law 34-65, regarding §3108 and §3202(a) Chapter 3, Title 19, Guam Code Annotated, effective May 8, 2018 our forms and applications for a Marriage License and Certificate has been updated. Previous editions are obsolete and will not be accepted. Please see below for downloadable forms and applications.
Divorces: The Office of Vital Statistics does not record, certify, nor issue divorce decrees. You may contact the Superior Court of Guam, Records Branch at (671) 475-3449. For divorces petitioned and settled outside of Guam, you may contact the respective state or country in which the order was finalized.
