GUAM FOOD CODE FORMS

The Division of Environmental Health (DEH) of the Department of Public Health and Social Services would like to inform the public that the new Guam Food Code (GFC) forms will be implemented effective June 15, 2016:

 

  • Form A: Application for Sanitary Permit;
  • Form B: Supplemental Application for Food Establishment;
  • Form C: GFC Variance Application;
  • Attachment C-1: Proposed Variance Form; and
  • Bare-hand Contact Application

The above forms are applicable to all retail food establishments, including Temporary Food Service Establishments.  Retail food establishments are establishments that serve food/beverage directly to consumers, such as restaurants, cafeterias, stall stands, coffee shops, bars, caterers, retail stores, bakeries, and delicatessens, and does not include food manufacturers, wholesalers, and distributors. 

In preparation for the implementation of the GFC forms, DEH has partnered with the Small Business Development Center (SBDC) to provide training in how to complete these new forms.  The training is meant primarily for individuals who may be interested in starting a retail food facility; however, we believe existing operators of such business may also benefit from the training.  Below is a schedule for the following courses:

May 13, 2016       8:30 am – 11:30 am

  1. Form A:  New Sanitary Permit Application
  2. Form B: Supplemental Application for Food Establishment

May 18, 2016       8:30 am – 11:30 am

  1. Form A:  New Sanitary Permit Application
  2. Form B: Supplemental Application for Food Establishment

June 08, 2016       8:30 am – 11:30 am

  1. Form C: Variance Application
  2. Attachment C-1: Other Proposed Variance
  3. Bare-Hand Contact Application
