Sanitary Permit Renewal Application

The Department of Public Health and Social Services would like to notify all establishments regulated by the department that Sanitary Permits for 2011-2012 expired on June 30, 2012.  If not done so already, businesses are instructed to submit their renewal applications immediately for the 2012-2013 permit.

The Sanitary Permit renewal application may be obtained from the Division of Environmental Health, located on the 2nd floor of the Mangilao Public Health Building, or it may be downloaded from

A copy of the 2013 Business License and the expired 2012 Sanitary Permit must be attached to the renewal application for categories of Eating and Drinking/Food Establishment; Laundry/Dry Cleaning; Hotel/Public Swimming Pool, Massage Parlor/Therapeutic Massage, and Tattoo Shops.

In addition to the documents noted above, the following establishment categories must submit the following supporting documents:

  • Institutional Facility – Copy of Childcare License
  • Beauty and Barber Shop – Copy of Cosmetological Establishment License
  • Hotel and Worker’s Dormitory – Copies of Worker’s Dormitory Permit; Contractor’s License for 2013; and TLUC Approval Letter if more than five H-2 Workers

To minimize the waiting time, renewing applicants paying by check may drop off their completed applications to the Division of Environmental Health, and return at a later date upon notification of its availability for pick up.  Unfortunately, similar arrangement cannot be provided for those paying by cash or credit card.
