Shriners Outreach Clinic

The Department of Public Health and Social Services (DPHSS) announces that the Medical Team from the Shriners Hospital for Children in Hawaii will be providing free consultative services for children up to age 18 who have Orthopedic conditions, burns, or spinal cord injuries.

Children with deformities resulting from disease and injuries involving the bones, joints and muscles, as well as “healed burns,” including loss of any part of the child’s body should be referred for evaluation to the Orthopedic Physicians of Shriners Hospitals for Children in Hawaii. Please contact our Department.

Patients will be seen by appointment only.  New patients are encouraged to obtain a referral and medical summary from their private physician, and must provide an updated shot record, a copy of their birth certificate, legal guardianship documents (if applicable), and a copy of their insurance card in order to be seen at the Shriners Outreach Clinic.  Returning patients are also reminded to provide X-ray films and/or laboratory results as previously instructed.  X-ray and/or laboratory orders can be picked up between 8:00am to 5:00pm, Monday through Friday at Medical Social Services in Room 152.

The Shriners Outreach Clinic will be held at the DPHSS facility in Mangilao, from January 8 through January 15, 2013 from 8:00am to 4:00 pm.

For an appointment and/or additional information, please contact Cindy Malanum at 735-7351/7174 or via email at maria.malanum@dphss.guamdev.com.
