Shriners Outreach Clinic
The Department of Public Health and Social Services (DPHSS) would like to announce that a medical team from Shriners Hospital for Children – Honolulu will be on Guam to provide free medical consultations for children, birth to 18 years old, with orthopedic conditions, spinal cord injuries, and burns. The Shriners Outreach Clinic will be conducted at Central Public Health in Mangilao on January 26-30, 2015 from 8 am to 4 pm. Patients will be seen by appointment only.
Parents of new patients are encouraged to obtain a referral and medical summary from their child’s private physician, and must provide a copy of their child’s birth certificate, legal guardianship papers (if applicable), an updated shot record, and a copy of their insurance card in order to be seen. Parents of returning patients are reminded to provide x-rays and/or laboratory results as previously instructed. X-ray and/or laboratory orders can be picked up at Medical Social Services Room 152, from 8 am – 5 pm, Monday to Friday.
