Treatment of Minor Patients

To ensure the safety of minor children, effective October 1, 2013, the Department
of Public Health and Social Services is requiring that parents or legal guardians
of children under 18 years old bring the following documents before their children
can receive medical treatment including immunizations:

(1)     Government-issued photo ID of parent or legal guardian
(2)     Child’s birth certificate (only parent listed will be allowed to sign for
        the child) or legal guardianship papers

If another person is bringing the child, a written authorization from the parent/legal
guardian is required.  This applies to all services unless exempted by law.  For more
